Transition to adulthood is a crucial stage in the lives of all young people and a time when those with autism may face particular challenges.
Transition planning, for those on Education, Health and Care (EHC) plans starts in Year 9 (aged 13/14) but not everyone with autism will have an EHC plan.
If a young person with autism is in mainstream school in Stockport, the Autism team in Children’s Services will offer an ADAPT (After Diagnosis Autism Planning Team) meeting in Year 11 (15/16) to support transition. This will be followed up by a letter outlining further support available.
