Transition and Continuing Healthcare (CHC)

Young people in receipt of Continuing Care and their families/carers will be supported in the transition to adult services from the age of 14yrs.  This typically starts through introduction at the year 9 school review.  The Continuing Healthcare Transition Nurse will attend key meetings regarding transition, working together with the (Continuing Care) Children’s Nurse and other relevant health and social care professionals until assessment for NHS Continuing Healthcare age 17yrs.  

This assessment will determine a decision in principle so that, wherever applicable, effective packages of care can be commissioned in time for the young person’s 18th birthday.
