Adult Community Therapy Team (ACTT)

The ACTT is a community rehabilitation service comprising of Physiotherapists, Occupational Therapists and Assistant Practitioners. They sit alongside and work closely with the Steady in Stockport Falls Service. The therapy staff work together with the patient, the local neighbourhood teams, other professionals, family and carers to provide person-centred, goal orientated care.

The aim of the service is to enable adults with a physical disability to achieve their optimum level of functional independence in all aspects of daily living, in their place of residence. This includes:

  • Advice on management of long term conditions for patients and their carers.
  • Assessment and intervention to improve mobility and reduce the risk of falls.
  • The service has a limited budget for provision of standard indoor walking aids. We have a walking aid clinic which is appointment only, at Kennedy Way in Stockport on Monday afternoons. Referrals can be made through a GP or other healthcare professional. Self-referrals are accepted for walking aids if the patient has been seen by ACTT within the last 6 months for the same problem.
  • Unfortunately we are unable to provide outdoor mobility aids.
  • Assessment and intervention to optimise independence in daily living activities, for example kitchen and self care tasks.
  • Provision of simple and complex activities of daily living equipment as part of a rehabilitation programme (from the Millbrook and local equipment stock)
  • Advice and demonstration of techniques for self-management.

Appropriate problems are identified and agreed goals are set and reviewed at each visit. Outcomes are based on the level of achievement of these goals.

Who is eligible?  Adults aged 18 years and above (school leaving age), with a physical disability who are registered with a Stockport GP (NB Neurological conditions are seen by the Community Neuro-Rehabilitation Service based at Beckwith House).

Written referrals are accepted from any health or social care professional.

It is expected that the referrer has had face-to-face contact with the patient, the only exception being in the case of a patient’s request for a walking aid.

The patient must consent to the referral and be aware that it has been made.

Access Criteria

  • Recent change in functional ability, skill level or social situation i.e. within the last 2 months.
  • Specific, measurable, time-limited goals.
  • Ability of the patient to co-operate with advice or intervention.
  • Domiciliary intervention only. The patient needs to require rehabilitation within the home environment. For example, unable to mobilise up/down stairs, transfer on/off bed, difficulty with meal preparation, outdoor mobility.
  • The team is unable to provide ongoing maintenance therapy.
