- Description
Transition is supported by:
- Each child is assigned a key person who is responsible to help them feel settled, happy and safe encouraging their development and learning.
- Introductory visits with the child and parent are in place to meet the key person and staff within the setting.
- Settling in sessions with a graduated approach (with parents present) enable children to build up to a full session.
- In the summer term Primary school teachers are invited in to see the children within our setting to aid transition. In some cases transition booklets are developed unique to the child.
Needs are identified, assessed and kept under review by:
- All children's needs are assessed individually and appropriate adjustments are made.
- Baseline assessments are made on entry then continually monitored, supported and tracked ensuring that they develop and learn as appropriate.
- The Key person raises concerns with the designated SENCO then liaises with the parents in identifying needs, deciding outcomes and planning for provision making sure targets are achieved.
- Play plans are drawn up and reviewed every 6 weeks.
- If little or no progress/targets are achieved then support from outside agencies is sought.
- If a referral for a single need or multiple needs is required then a DNRF is completed along with the parents.
- Parents are encouraged to refer their child for an Education Health and Care Plan assessment if necessary.
Reasonable adjustments made within the setting are:
- Fully inclusive practice is based on individual requirements where children are integrated into small groups with, if necessary, one to one support with the designated SENCO, building on what the child already knows including different approaches to learning.
Consultation/encouragement with parents:
- All parents have an opportunity at drop off and pick up times to talk about any concerns they may have which is always encouraged. The Leader manager is available at these times or through pre arranged appointments.
- An open door policy is in place, support plan review meetings are held and home visits are done if required. Parents are also signposted to local support networks.
Staff training: - All staff have a secure knowledge of the EYFS framework whilst helping the child to learn new skills acquiring knowledge and developing their understanding through the 7 areas of learning and development.
- Training courses/ INSET days and sharing good practice/ideas are encouraged through a variety of avenues: Learning leads, Learning support service, in house training sessions (developed from good relations with outside agencies), Educare online training company.
- The designated SENCO and Deputy are actively involved in SENCO network groups and attend regular meetings and forums.
Setting specialisms:
- The designated SENCO provides provision for all children to be inclusive in the setting.
- All staff currently hold a First Aid certificate.
