Children’s Community Learning Disability Team (CCLDT) and Healthy Young Minds Learning Disability Team (HYM-LD)

CCLDT and HYM-LD are two services working integrally as one team to support children and young people aged 3 to 18 years who have a moderate to severe learning disability with complex additional needs such as, but not limited to:

  • Behaviour that challenges e.g. aggression to others, self-injurious, destructive behaviours
  • Mental health difficulties/ distress e.g. anxiety, low mood
  • Low self-esteem
  • Complex continence difficulties including faecal smearing
  • Sleep difficulties
  • Severe difficulties adapting to puberty and adolescence

The services that the team provide to individual children, young people and parents/ carers are based on an initial assessment which usually takes place at the family home. This information informs the practitioner that the child is or is not eligible to receive a service from the team and which part of the service and type of intervention will best meet the needs of the child, young people and family /carers. The interventions that the team provides are:

  • Webster-Stratton Specialist Incredible Years 14 week course for parents and carers.
  • Sleep assessment and intervention programme/sleep workshop.
  • Positive Behaviour Support 8-week Nurture Programme for parents and carers.
  • Functional Analysis and Positive Behaviour Support Plan
  • 1 to 1 Community Nursing, intervention dependent on the outcome of initial assessment e.g. support with continence, communication
  • Psychological therapies and approaches
  • Riding the Rapids 10 week specialist course for parents and carers (primary and secondary)

In order to make a referral contact the team to request a referral form. Please be clear in your referral about what help you are asking for and what the difficulties are. If you are unsure about whether the child or young person is eligible, phone one of the team to discuss. A full and detailed copy of the eligibility criteria can be e-mailed to you. Referrals should be made by a professional involved in the care of the child/ young person e.g. consultant paediatrician, educational staff, health visitor, social worker etc.

Referral Details

Referrals to CCLDT should be submitted on the referral form by a professional involved in the child/YP care, which can be accessed by contacting the team at ccldt@stockport.nhs.uk or telephone to speak to one of the nurses 0161 419 2129/2112/2137. If you can please attach other relevant information for example SALT report, EHCP.
